Mission
Statement and Purpose
The mission of the Foundation is to support the community
programs of the Des Plaines Healthy Community Partnership by
generating and distributing financial and community resources.
What is the purpose of the Des Plaines Healthy Community
Partnership Foundation?
The Foundation was created to raise funds to identify, link and
mobilize community assets that will benefit all people of Des
Plaines. These funds are not available through local tax revenues.
Where do the funds come from?
Foundation funds are obtained from individual, business, and
corporate tax-deductible contributions, as well as, from other
foundations.
Contribution Form
Who decides how the money will be spent?
All donations to the Foundation are spent with the approval of
the Board of Trustees. The use of the funds must be consistent with
the Foundation’s mission and goals.
